The Homeless Prevention Center is seeking a dynamic individual for the position of Service Coordinator in our Family Supportive Housing program. The Service Coordinator functions as part of a two-person team to provide intensive housing case management and service coordination to families who are homeless or are at-risk of homelessness. Services are typically provided for a period of two years – longer as appropriate – while a family stabilizes in housing. The Service Coordinator works collaboratively with families and our community partners to: provide resources and support to clients; assist families in developing life skills; and facilitate access to resources that lead to permanent housing and a positive quality of life. The ideal candidate has a desire to work in a strengths-based environment and a willingness to empower people to overcome housing and employment barriers. Strong case management experience and a degree in Education, Human Services, Social Work, Counseling or related field is preferred. - Family Supportive Housing Service Coordinator
Homeless Prevention Center, Rutland, Vermont Full-time Salary: $33,000 to $35,000 /yearPrincipal Duties:
Provide support to between 12 and 15 clients as part of the FSH caseload. - Meet regularly with clients – both inside and outside of HPC offices – to assess housing needs and complete intake, following grant protocol guidelines.
- Provide training and referrals to clients in the areas of financial empowerment, life skills, tenant education, parent and child resiliency, and support in addiction recovery.
- Assist clients in the preparation of Individual Service Plans (ISPs) to determine their goals and assist with self-advocacy in relation to housing and family domains.
- Engage in advocacy and collaboration with agencies on behalf of families.
- Coordinate parent/child education and community outreach on issues of homelessness.
- Coordinate various plans among appropriate agencies, particularly with Family Services and Reach-Up, and conduct general advocacy with a focus on successfully obtaining permanent and safe housing for clients and their children.
- Develop opportunities for employment for participants in collaboration with organizations providing employment supports, such as Vermont Department of Labor and Creative Workforce Solutions.
- Identify and access additional appropriate resources for families.
- Attend mandated training, and be available for additional training opportunities as identified by the Executive Director.
- Work collaboratively as part of a Team.
- Share the responsibility of an on-call rotating schedule for issues that arise.
- Perform any other duties as specified under the mandates of the FSH grant.
Administrative Duties: - Maintain appropriate records as required.
- Data entry and preparation of reports as assigned.
- Review and development of needed additional services.
- Attend local housing and other meetings as assigned.
- Work with partners such as NewStory Center in general support of issues surrounding DV and SV.
- Perform any additional administrative or other duties as needed.
Qualifications: A Bachelor’s Degree in Education, Human Services, Counseling, or related field, with some direct experience working in human services, particularly with families and children. Must be detail-oriented able to maintain accurate records and prepare comprehensive progress reports, as well as develop and track self-identified goals. Should have knowledge and experience working with people struggling with poverty and substance abuse. Must have the ability to work with various agencies within the community, and demonstrate the ability to work collaboratively as part of a team. Must have the ability to communicate and work effectively with a wide range of people, and should demonstrate a non-judgmental and compassionate attitude towards clients, focusing on strengths. The Service Coordinator will have knowledge of resources in the community along with the ability to teach simple life skills, including budgeting and personal safety skills. This person will demonstrate the highest ethical standards and be able to communicate effectively the goals of the Family Supportive Housing Program. Must have access to reliable transportation, possess clean driving record and pass required background checks.
