Coordinated Entry/Housing Support Case Manager
Springfield Supported Housing Program(SSHP), is a private non-profit human service organization located in Springfield VT, which provides housing services to families and single adults who are homeless or at risk of homelessness. SSHP manages 13 transitional housing units where we provide home based services. We work with a diverse population that are facing life challenges in addition to homelessness: childhood trauma, generational poverty, substance abuse, mental illness, domestic violence, and inadequate education are some of the issues facing our program participants.
Position Summary
The Coordinated Entry(CE), and Housing Support Case Manager(HSCM) works closely with the Executive Director(ED) and the Assistant Director(AD) on CE and HSCM.
CE Responsibilities
- CE is responsible for intake/assessment of ALL agency referrals
- Meet Weekly with Case Managers to discuss clients and prioritized need
- Record and report to all involved parties using state provided database
CE Accountabilities
- CE will meet monthly with local Housing Review Team staff
- CE will meet monthly with Coordinated Entry team around changes to process
- CE will prepare and maintain prioritized list
CE Competencies
- Strong interpersonal skills are essential.
- Strong knowledge of navigating our community referral partners
- Computer knowledge and ability to maintain accurate data base
- Candidates must have a valid driver’s license and a dependable vehicle.
- Ability to set boundaries and provide self care on a regular basis
HSMC Responsibilities
- HSCM to provide housing placement and ongoing support to individuals facing life challenges such as homelessness, substance abuse, mental illness, etc.
- Meet Weekly with Case Managers to discuss clients
- Record and report to all involved parties using state provided database
- HSCM is responsible for intake/assessment and must be able to interact positively with landlords and other service agencies, and is responsible for regular, at home visits with program participants
HSMC Accountabilities
- HSCM will meet
- Files will be reviewed quarterly for completion and accuracy
HSMC Competencies
- Strong interpersonal skills are essential.
- Strong knowledge of navigating the local recovery/treatment system
- Computer knowledge and ability to maintain accurate data base
- Candidates must have a valid driver’s license and a dependable vehicle.
- Ability to set boundaries and provide self care on a regular basis
- Qualifications
BA preferred with prior experience in human services.
3-5 years experience working in a human services field is desirable.
The start date is May 1, 2019
Compensation
Salary commensurate with experience
15 days vacation / 5 sick days yearly/ 12 Holidays when falling on work days
