Vermont Coalition to End Homelessness October 15th Monthly Meeting Minutes – DRAFT


VOTE WARNED (to be voted on November 19th)

Coordinated Entry Assessment Update & Youth Coordinated Entry Policies & Procedures

Additions to Complex Service Needs to be more inclusive in capturing the needs of youth and young adult households.

Other changes to the assessment:

    • Client Contact Info Option added
    • Client Certification Option added
    • Other language clarifications in Assessment Questions related to homelessness, past evictions, and experience of non-intimate partner violence
  • New CE Data Standards – new “pick lists” for prior housing situation
  • CE Policies and Procedures that address the above, and prioritization for transitional housing.
  • Youth Action Board looked at the new questions to make sure that youth are involved in the planning process. They felt excited about the IEP/504 plan and that these questions could help open access to youth and are youth centric.

Point-in-Time Paper Form update 2020 Comments to this form can be made to Andrea by November 29th.

What Changed:

  1. No HUD Changes for this year!
  2. Decluttered top instructions & Added Minimum Information Requested
  3. Section 1 will be editable: Volunteers can fill this out before printing.
  4. Arrows added to indicate that there is a question that needs to be checked off and/or filled out
  5. Question added for making an Observation
  6. Currently Fleeing Violence was updated to reflect ALL fleeing of violence and not just from an intimate partner.
  7. Section 3: Age Ranges changes to reflect the current ranges <18, 18-24, 25-59, 60+).
  8. Section 3: Age Range column added to form

Administrative box at top of form for those collecting/entering these forms to indicate completed.
