Save the Date: Webinar – Effective Coordination between Hospitals and CoC Homeless Assistance Providers – November 10, 2015 – 1 PM EDT

This webinar will explore how three hospital systems – state, county and private nonprofit – have streamlined their internal operations, coordinated with homeless Continuums of Care and effectively leveraged both internal and external resources to reduce overuse of hospital emergency departments and frequent readmissions, while improving discharge outcomes and expanding access for homeless and at-risk patients to permanent housing and other needed community-based resources and services.

  • Lisa Bahadosingh, Community Liaison for Regional Initiatives, Fairfield County, CT
  • Joyce Platz, Executive Director, Residential Services, St. Vincent’s Hospital, Bridgeport, CT
  • Corrin Buchanan, Program Manager, Housing for Health, LA County Health Services, Los Angeles, CA
  • Lynn Kovich, Senior Consultant, Technical Assistance Collaborative, Boston, MA

Who Should Attend?

This webinar primarily targets agencies administering HUD-funded CoC and HOPWA housing and supportive services. It will be particularly relevant to administrators and program planners responsible for developing agency-specific protocols and those who participate in community-level systems planning and development.

  • This webinar is also open to the following U.S. Department of Health and Human Services (HHS) grantees due to their key role in collaborating with HUD’s grantees to address the healthcare needs of people experiencing homelessness:
  • Health Resources and Services Administration’s (HRSA) Ryan White and Health Center grantees;
  • Administration for Children and Families’ (ACF) Runaway and Homeless Youth Program grantees;
  • Substance Abuse and Mental Health Services Administration’s (SAMHSA) Targeted Homeless and HIV Programs grantees.

Learning objectives include:

  • Sharing innovative approaches at the state and local levels for connecting hospital and CoC resources to improve care and reduce costs for homeless and at-risk patients.
  • Sharing information about successful solutions being implemented to leverage Medicaid and local resources to fund permanent housing interventions.
  • Engaging with front-line experts who can offer access to cutting-edge information and suggest workable solutions for improved hospital discharge outcomes through braiding federal, state and local resources.
